Today's Yeh Hai Chahatein 21st March 2023 episode starts with Nayantara putting Prem to sleep and Samrat quietly entering the room after some time.

However, his foot bumps into something hard as he shouts in pain.

Nayantara wakes up, asking him to be quiet, gradually creating a mild conflict between them.

Nayantara asks Samrat to go and sleep with Mansi, explaining that Samrat won't feel tortured that way.

However, Samrat retorts, saying that he doesn't want her to prove him wrong in court and will continue to stay with her and Prem.

As Nayantara goes to sleep, Samrat quietly thinks about how he is faking getting irritated with Nayantara, wanting to throw Mansi out.

At midnight, Nayantara wakes up and slowly takes her bag and torch, sneaking out of the room.

She meets Ishani and then they drive to Dr. Anushka's clinic. 

Ishani distracts the guard by asking about a fake morning society and Nayantara sneaks into the clinic only to find the door locked. 

She then finally unlocks it by using a pin and searches for Mansi's files amongst the pile of files. She soon finds them and gets shocked to see that Mansi is pregnant. 

Meanwhile, Mansi sneaks into Samrat's room and sees Nayantara missing. She understands how Nayantara is so desperate to investigate her reports, remembering Nayantara's nature.

She remembers asking the doctor to change her reports to the reports of a 3-month pregnant woman, to which the doctor agrees.

On the other hand, Nayantara feels guilty as she drives home with Ishani. Nayantara goes to pick up Prem from school the next day.

As she waits, she sees a boy almost getting hit by a car and quickly manages to rescue him.

Interestingly, the boy's mother is revealed to be Mansi's doctor who thanks her.

Nayantara requests her to tell the truth, telling her about Mansi who does not care for Prem and such a person getting custody of Prem in a few months will only create havoc. 

The doctor understands Nayantara's situation and discloses that Mansi has never been pregnant for the last few months.

However, the doctor requests Nayantara to not reveal the truth to Samrat, fearing that her licence would get cancelled.

Nayantara promises that she won't let Samrat harm her and asks the doctor to help her as a mother. 

Nayantara requests the doctor to help her, being a mother herself.

As they hug each other, Mansi watches and spots them from her car behind.

After Nayantara goes away, Mansi calls her doctor and asks about her whereabouts. 

The doctor quickly lies, saying that she is at her clinic and did not meet Nayantara the second time. 

She asks Mansi not to worry because she has exchanged the files with that of a pregnant woman. 

Mansi understands that the doctor is lying and is determined not to let Nayantara reach Samrat to tell him the truth.
